If you want to add ConstantContact form in AMP then first you need to purchase and install Opt-in Forms for AMP plugin.
After installing you will find this option in WordPress Dashboard -> AMP optin -> Add New optin -> select “ ConstantContact ” option -> now you need five things – Form ID, List ID, Hidden ID 1 and Hidden ID 2.
Follow the simple steps to get all this ID’s from ConstantContact Account.
Step 1: First of all Signup/login at ConstantContact.
Step 2: Go to Constant Contact -> Form Designer Copy the shortcode and paste it in the post/page as per your requirement.
Step 2: Paste the shortcode in the post/page as per your requirement
Step 3: Go to the WordPress Admin Area -> AMP option panel-> AMP optin -> Position select the required opt-in.
Step 4: Inspect the form in browser and Copy the code and paste it in Notepad.
Step 5:Copy the Form URL and Paste this Form URL in AMP Opt-in form
Step 6: Copy the Form ID and Paste this Form ID in AMP Opt-in form
Step 7: Copy the List ID and Paste this List ID in AMP Opt-in form
Step 8: Copy the Hidden ID 1 and Paste this Hidden ID 1 in AMP Opt-in form
Step 9: Copy the Hidden ID 2 and Paste this Hidden ID 2 in AMP Opt-in form
Step 10: Now, select ConstantContact from the Drop down and enters the API URL and copy the shortcode and place where you want to display the ConstantContact Form.

Make sure you need to fill this form and then click on the submit button after that you will get the “Subscription successful” message.
