If you want to add Mautic form in AMP then first you need to purchase and install Opt-in Forms for AMP plugin.
After installing you will find this option in WordPress Dashboard -> AMP optin -> Add New optin -> select “ Mautic” option -> now you need three things – Form URL, Form Id, Form Name, Email ID .
Follow the simple steps to get all this ID’s from Mautic Account.
Step 1: First of all Signup/login at Mautic.
Step 2: After Signup/login Mautic clicks on the Components -> Forms option -> click on “New” button.
Step 3: Here you need to Select New Standalone Form
Step 4: Click on Details tab and enter Form name here.
Step 5: Click on Fields tab here you need to create fields and after that click on “Save & Close” button.
Step 6: Here you need to click on “Manual Copy” button.
Step 7: Copy the code and paste it in Notepad.
Step 8: Copy the Form URL and Paste this Form URL in AMP Opt-in form
Step 9: Copy the Form ID and Paste this Form ID in AMP Opt-in form
Step 10: Here you need to enter Form Name like below the screenshot
Step 11: Copy the Email ID and Paste this Email ID in AMP Opt-in form
Step 12:Create a new AMP Opt-in
Step 13: Now, Select Mautic from the Dropdown and paste the Form Id, Form Name, Email ID and place where you want to display the Mautic Form.

Make sure you need to fill this form and then click on the submit button after that you will get the “Subscription successful” message.
